Parent's Request for Driver's Education

 be enrolled in your state-approved driver education with the understanding that he/she will be under Bay State

Driving School's supervision during the Classroom portion and/or the Behind-the-Wheel phase of the program.

Please bring a copy of your child's birth certificate to the orientation day.  Return this pre-egistration form along with $10.00 to reserve your seat on the date you checked above. Classroom seats are not guaranteed unless paid in full prior to the scheduled class. Send or bring a copy of Learner's Permit or Birth Certificate.

*** Students do not have to have a Learner's Permit in order to enroll in their 30 hours Classroom Instruction.  He or she may take this portion of driving education once he/she reaches 15 years and 9 months of age prior to the first day of class. 

If you are registering for the classroom and/or behind-the-wheel (BTW) instruction, come prepared to pay on/or before the Orientation Day
