      I, _______________________________________________ agree to pay Bay State Driving School $45. per Behind-the-Wheel (BTW) hour if taking less than 6 hours of driving. 

      I, _______________________________________________ agree to pay Bay State Driving School $630. for 12 hours of Behind-the-Wheel (BTW) training, and 30 hours of classroom instruction.  Additional driving hours will cost $40. if necessary.  If I am paying for the classroom portion only, the cost is $250.  If only paying for 12-hours BTW, the cost is 500.

      I, _______________________________________________ agree to pay Bay State Driving School $240. for a package of 6 hours of Behind-the-Wheel (BTW) training.

      I, _______________________________________________ agree to pay Bay State Driving School $55. per hour for manual transmission (stick shift) training.

There will be no refunds after the first day of instruction.  Certified, personal check, money order, and cash accepted as form of payment.  There is a $35. fee for returned checks. 

Driving time is scheduled by calling or visiting our office.  It is the student's responsibility to be at the appointed time and place for all driving instruction.  There is a $45. fee for no-show if he/she does not show-up on time for his/her driving appointment or if he/she has not given at least 24-hour notice of cancellation to BSDS.  This fee must be paid before the student is scheduled to drive again.

Completion of driver training instruction does not guarantee qualification for a driver's license.  Nevertheless, by taking this course you may qualify for a lower car insurance rate.

Please mail a copy of your Learner's Permit along with this Registration Form and Payment.  Classroom seats and/or BTW instruction are not guaranteed unless paid in full prior to the scheduled class. 

*** Students do not have to have a Learner's Permit in order to enroll in the 30 hours Classroom Instruction.  He or she may complete this portion of driving education and then go to the RMV for the Learner's Permit.
